Position Summary:
The Condition Management Health and Wellness Program Coach supports the clinical department by cultivating behavior and healthy lifestyle change for participants enrolled in Asset Health’s coaching programs through participant-centered coaching, motivational interviewing, and behavior change strategies. The Health Coach assesses the participant’s current needs and readiness to change to deliver appropriate education, support, and guidance to facilitate healthy lifestyle change to prevent or manage chronic conditions. Additionally, the Condition Management Health Coach is responsible for assisting participants to explore personal “best health” vision and purpose, set aligned SMART goals, encourage progress, and overcome barriers which may interfere with reaching established goals.
Essential Duties & Responsibilities:
- Telephonic (audio and visual) health coaching, which includes contacting eligible and enrolled participants in one or more of our coaching programs, establishing a call schedule that meets the needs of the participant, and ensuring all efforts are made to contact participants within client specific guidelines
- Responsible for understanding the specific program and following the guidelines of each client
- Support health promotion and wellness programs aimed at encouraging members to adopt healthy lifestyles by actively participating in wellness activities
- Provide assessment, health education, and behavior change counseling using a collaborative approach to inspire members to build healthy habits
- Partner with cross-functional teams to aid in the design and or presentation of live events and special programs for in-person, virtual, or hybrid attendance
- Coordinate information to clinical team partners
- Maintenance of confidentiality and privacy of protected health information (PHI) utilizing safeguards as described under the HIPAA guidelines and inputting information into multiple documentation systems
- Must stay within health coach training and scope of practice – when necessary, refer to internal client resources such as EAP or other programs and/or external resources such as health care provider
- Continual education and/or learning on health behavior change theories, counseling, interviewing techniques, strategies and application required
- Must adhere to quality and ethical of standards of Asset Health Inc. coaching programs
- Leads with humility, charity, perseverance, relationships, engagement, and responsibility to create an inspiring, fun, and healthy work environment
- Performs other administrative and operational tasks as required
- Collaborating with in in-house creative team to amplify nutrition offerings and resources
- Reviewing content (ie. Resources, courses, webinars, etc.) to meet industry standards
Education and/or Work Experience Requirements:
Required:
- A bachelor’s degree or higher in health and wellness-related field (health education, health promotion, nutrition, exercise science/physiology, nursing, counseling, etc.)
- Health coaching certification from a National Board for Health and Wellness Coaching (NBHWC) approved training program on health topics including nutrition, exercise, alcohol, tobacco cessation, stress reduction and resiliency training, weight management, cholesterol, hypertension, chronic conditions, etc.
- Registered Dietician
Preferred
- NBHWC Board Certified
- Minimum 3 years of equivalent experience coaching on health topics
- Training/education in behavior change, motivational interviewing
- Specialty certifications/training in personal training or fitness, nutrition registered dietitian, or tobacco cessation
- Telephonic coaching experience
Knowledge, Skills and Competencies:
- Strong interpersonal, social, and communication skills
- Ability to create strong, trusting, professional relationships with participants to encourage engagement and participation in health/wellness coaching programs
- Knowledge of the principles of behavior change, adult learning, group facilitation and of basic health, nutrition, and fitness principles
- Understanding of primary, secondary, and tertiary prevention strategies
- Foster growth mindset and openness to feedback
- Passionate about supporting individuals towards the best version of themselves and progressing towards their “best health”
- Excellent verbal and written communication, computer, and documentation skills required (e.g., Microsoft Office Suite)
- Ability to research academic and peer-reviewed studies on health promotion and wellness
- Must possess strong time management skills in order to effectively manage participants within client guidelines and ensure timely documentation
- High degree of patience, emotional control, creativity, detail, and extroversion
- Must be confident, assertive, optimistic, empathetic, and objective
- Comprehensive understanding of risk factors, biometrics and behaviors that contribute to chronic disease
- Ability to work independently or in a team setting with a high energy level
Working Conditions and/or Physical Requirements:
- Flexible availability, occasional work during evenings (5-9p)
- Prolonged periods sitting at a desk and working on a computer
- Must have access to secure internet network and a private, quiet room to conduct coaching calls
- No manual labor or lifting
- This can be a remote/work from home position
