Job Description

The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

at least 2 years of Case Management experience
Case Manager certificate is NOT required

8A-5P + 40 hours/week + 13 weeks

$55/hour - $2,200/week

Start Date: 5/30/2025

VIRTUAL Training Schedule: 2 weeks, Mon-Fri, 8:00a-5:00p
